Do I Need a Psychiatrist or Psychologist?

Psychiatrist and psychologist are frequently confused. The distinction matters clinically because the two professionals have different training, different scopes of practice, and different clinical roles.

The Psychiatrist

A medical doctor with postgraduate specialisation in psychiatry; in India, MBBS followed by MD or DNB Psychiatry, nine or more years of medical training. Core competencies: psychiatric diagnosis; medical evaluation of psychiatric presentations; prescription and medication management; co-occurring condition management; comprehensive treatment planning.

The Psychologist

A clinical psychologist in India holds an M.Phil in Clinical Psychology (RCI-recognised) following a postgraduate psychology degree, two years of supervised clinical training. Core competencies: structured psychological assessment using validated instruments; evidence-based psychological therapy delivery (CBT, DBT, ERP, ACT, psychodynamic); neuropsychological profiling. Cannot prescribe medication and does not conduct psychiatric diagnosis in the medical sense.

Which One First?

Start with a psychiatrist when: the diagnosis is uncertain; medication may be required; physical symptoms accompany psychological ones; prior therapy has not produced adequate improvement; or the presentation involves significant functional impairment.

Start with a psychologist when: the diagnosis is clear, the presentation is suited to structured psychological intervention, medication is not indicated, and a referral pathway to a psychiatrist exists if the clinical picture changes.

The most common error is starting with a psychologist or therapist when a psychiatrist should be first, producing delayed treatment of medical components that, for conditions where those are significant, has real consequences.

How They Work Together

The most effective treatment for complex psychiatric presentations involves both: the psychiatrist providing diagnostic accuracy, medical evaluation, and pharmacological management; the psychologist delivering structured psychological intervention matched to the specific diagnosis. Genuinely complementary components of a comprehensive treatment plan.
